At Caring Minds Health Services LLC, our Notary Services are available to individuals, families, and staff who need documents notarized accurately, promptly, and in compliance with state laws.

• DDD-related consents and documents
• Employment and HR-related forms

Requirements for Notarization

• Valid, government-issued photo ID (e.g., driver’s license, state ID, passport)
• All signers must be present and willing at the time of notarization
• Original (unsigned) documents if signature must be witnessed
• Documents must be complete (no blank sections)
